Manchester City Council’s Directorate for Children and Families has recently asked us to share this PIP questionnaire from DWP, which is for people who have previously claimed Personal Independence Payment (PIP) to give feedback on how they found the process. The questionnaire has only six questions and could be completed in five minutes. No personal information is asked for in the form.

Please complete the form and share with your friends who have accessed PIP in the past, if possible. The Department for Work and Pensions (DWP) have said that feedback of people’s experiences will influence on-going and future claims so it would help to get as many responses as possible.
